1. GENERAL Credit Card Charge Form (use it to book accommodation in Moscow & St Petersburg, transfers, excursions, tours and extra services)
2. TRAIN TICKETS (use it to book train tickets)
3. VISA SUPPORT: INDIVIDUAL (goes in addition to the first form to indicate visa support data ONLY)
4. VISA SUPPORT: GROUP (goes in addition to the first form to indicate GROUP visa support data ONLY)
